Description

Przedmiotem zamówienia jest żywienie pozajelitowe na okres 12 miesięcy. Szczegółowe wymagania dotyczące realizacji zamówienia zostały określone we wzorze umowy oraz formularzu asortymentowo-cenowym stanowiącymi odpowiednio załącznik nr 5 do SWZ i załącznik nr 2 do SWZ. Wspólny Słownik Zamówień CPV: 33.69.22.00-9 produkty do żywienia pozajelitowego. Zamawiający dopuszcza możliwości składania ofert częściowych (2 - zadania), zgodnie z Zał. nr 2 do SWZ stanowiącym formularz asortymentowo – cenowy. Zamawiający nie określa liczby zadań, na którą wykonawca może złożyć ofertę lub maksymalnej liczby zadań, na które zamówienie może zostać udzielone temu samemu wykonawcy. Zamawiający dopuszcza możliwość składania ofert częściowych na dowolną ilość zadań.
