Manchester City Council (MCC) are seeking to commission a service provider to deliver a citywide adult's nutrition support service and to design and deliver a citywide, evidence based accessible multi-component, lifestyle weight management service (tier 2) for children, families and adults, in the community. To deliver a comprehensive citywide nutrition support service for adults aged over 18 years, identified as malnourished or at risk of becoming malnourished, in order to improve or maintain their nutritional intake. To design and deliver a citywide, evidence based accessible multi component lifestyle children and family weight management service in the community (tier 2), that will assist children and young people between 2 and 18 years of age, who have a Body Mass Index (BMI) equal to or greater than the 91st centile to reach and maintain a healthier BMI. To design and deliver a citywide, evidence based accessible multi component lifestyle weight management service for adults aged over 18 years of age, in the community (tier 2), that will support people with a BMI equal to or greater than 25 kg/m², to lose weight and learn how to maintain a healthier weight. To update and develop new care pathways to support all functions of the service, in partnership with other professionals/organisations. To design and deliver training that supports the functions and pathways of the service, for health and social care professionals and clients.
