A review of the recommendations for the physical dosimetry of Boron Neutron Capture Therapy (BNCT)

For the development and clinical implementation of the particular type of radiotherapy BNCT, it is of crucial importance that the basic characteristics of the neutron beam free-in-air and of the basic dosimetry quantities in a phantom are determined in a coherent and reproducible way. Consistent dosimetry is a requirement for a reliable comparison with conventional radiotherapy or other treatment modalities and for the safety of patient treatments. This imposes the condition that the basic dosimetric methods must be traceable by international standards. A consortium consisting of most of the BNCT groups in Europe has produced recommendations and guidelines for BNCT dosimetry. The recommendations presented in this report are based on the available knowledge and experience gained so far, including theoretical reviews, systematic experimental comparisons and analysis of the methods at all available European BNCT facilities. Criteria for the selection of the methods and procedures are defined and applied in the recommendations. The recommendations are limited to the basic experimental procedures related to dosimetry prior to clinical treatment, including quality assurance procedures. Guidance on treatment planning and other clinical and biological issues are not the subject of this document. Recommendations are given for: - Beam characterisation, which includes primary and supplementary methods to determine the free-in-air beam profile of neutrons and photons, to determine the neutron and photon profiles in a phantom and to determine the neutron and photon energy distributions. - Beam monitoring, being essential for the accurate dose delivery. - Beam calibration, which includes the definition of the reference conditions, primary and supplementary methods for the determination of the neutron and photon absorbed dose and a review of corrections that are related to dosimetry in mixed fields of neutrons and photons. Although the recommendations are focused on measurement in well-defined reference conditions, dosimetry for non-reference conditions is reviewed for completeness as well. Finally the quality assurance and the uncertainties in the measured quantities are estimated. The report demonstrates the feasibility to establish a collaborative network on a BNCT dosimetry-standard in Europe, with links to North, South America and Asia.
