Poland-Gdansk: Antithrombotic agents
1. The subject-matter of the contract is the supply of medicinal products as part of the final import for a period of 12 months in the quantity, range and requirements set out in Annex 6 to the ToR.
2. The contracting authority shall allow partial tenders to be submitted. Partial tender means the individual items set out in Annex 6 to the ToR. The contractor may submit a tender for one or more items.
3. The contracting authority does not agree to amend Annex 6 to the ToR – the product and price form, failing which the tender will be rejected.
4. The contracting authority specified the method of packaging in the product and price form. On items where no such description is available, the Contractor may offer any form of packaging. The contractor must value the packaging method proposed with appropriate conversion. In the case described above, the Contracting Authority agrees to amend Annex 6 to the ToR, the product price form in the quantity column (possibility of adding a new column with the proposed conversion) and a column of one measure.
5. Execution of sub-orders no later than 21 days after receipt (by fax or e-mail) by the Contractor of an application for final importation with the consent of the Minister for Health.
6. The contracting authority shall not allow variants to be submitted.
7. The contracting authority does not provide for the award of supplementary contracts.
