Description

1. Przedmiotem zamówienia jest dostawa wyrobów jednorazowego użytku, których szczegółowy opis, asortyment, szacunkowe ilości oraz wymagania określone zostały w Załączniku nr 7 do SIWZ.
2. Niepełnienie chociażby jednego z wymagań określonych w pakietach wskazanych w Załączniku nr 7 do SIWZ spowoduje odrzucenie oferty w danym niespełniającym wymagań Pakiecie.
3. Zamawiający wymaga, aby zaoferowany asortyment spełniał wymagania dotyczące wyrobów medycznych zawarte w ustawie z dnia 20.5.2010 roku o wyrobach medycznych (tj. Dz.U. z 2015 r., poz. 876 z późn. zm.) oraz w wydanych na podstawie tejże ustawy, aktach wykonawczych.
4. Dostawa wyrobów jednorazowych będzie realizowana na podstawie zamówień częściowych. Maksymalny termin dostawy: 3 dni robocze (z wyłączeniem sobót) od daty złożenia zamówienia składanego faksem, telefonicznie lub drogą e-mail przez Zamawiającego. Jeżeli dostawa wypada w dni wolne od pracy lub sobotę dostawa nastąpi w pierwszym dniu roboczym po wyznaczonym terminie
