Description

1. The subject of the contract is the successive supply of consumables for microsurgery of the eye for the Associate 6700 and EVA 8000C0M5 (Scope I), lenses and preparations for ophthalmic treatment (Scope II) and other consumables for eye microsurgery (Scope III). A detailed description of the subject matter of the contract is set out in Annex 8 to the ToR.
2. Delivery of the subject matter of the contract for the Ophthalmic Operational Block (Scope I and III)/Apteki (Scope II) of Dr Karol Jonscher Municipal Medical Centre in Łódź (ul. Million 14, 93-113 Łódź) will be carried out at the Contractor’s expense and risk.
