1. The subject of the contract is the supply of dressings of specialized medical materials, small medical equipment, medical equipment, training equipment, stretchers, phantoms and trainers for 34. WOG and for military supply units and institutions. 2. The subject of the contract is divided into the following 8 lots: 1) Part 1: Supply of specialist dressings; 2) Part 2: Supply of medical supplies; (3) Part 3: Supply of small medical equipment; 4) Part 4: Supply of medical equipment; (5) Part 5: Supply of training equipment; 6) Part 6: Delivery of stretchers; 7) Part 7: Delivery of phantoms; 8) Part No. 8: Delivery of trainers. 3. The detailed quantitative specification shall specify: 1) Annex 1A to the FTZ – Part 1; 2) Annex 1B to the FTZ – Part 2; 3) Annex 1C to the FTZ – Part 3; 4) Annex 1D to the FTZ – Part 4; 5) Annex 1E to the FTZ – Part 5; 6) Annex 1F to the FTZ – Part 6; 7) Annex 1G to the FTZ – Part 7; 8) Annex 1H to the FTZ – Part 8; - depending on the parts to be applied for by the Contractor. The Contractor shall deliver the goods to the Ordering Party's medical warehouse located at the following address: Rzeszów ul. Lwowska 5 (in hours: 7.30 – 13.00), on the Employer’s working days (i.e. Monday to Friday, excluding public holidays) at its own expense and risk. 5. The delivered goods will be: a) brand new, b) packed in original packaging marked with the name of the manufacturer / importer and a description of the content, c) individually packed in internal, airtight packaging preventing contact with the atmosphere, moisture, etc. During transport and storage – this applies in particular to materials that may lose their properties under the influence of air, (d) will be delivered in packaging protecting against quantitative and qualitative changes, (e) where part of the subject-matter of the contract requires repackaging, the subject-matter of the contract must be delivered in protective packaging. 6. At the request of the Ordering Party, the Contractor is obliged to submit declarations or certificates of conformity of the goods, issued by an authorized entity, in accordance with the requirements set out in the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620, as amended). 7. The contractor who is the producer of the goods is obliged throughout the term of the contract to have an up-to-date administrative decision for conducting business, issued by the body of official medical and pharmaceutical control. The above-mentioned decision must specify the type and scope of activities carried out and the type of equipment and materials being produced and marketed, in accordance with the applicable legal provisions in this regard. 8. The Contractor will provide with the subject of the contract documents in Polish containing all the information necessary for the direct user about the assortment, including the method of its storage. In the event that the subject of the contract requires the inclusion of technical documentation, e.g. proof of equipment, technical passport, instructions for use, etc., the Supplier is obliged to provide them together with the subject of the contract. The technical documentation must be made in Polish and completed by the manufacturer or Supplier in an appropriate manner. If the subject of the contract requires documentation in the form of a warranty card, the Supplier is obliged to provide the warranty cards in Polish and completed and sealed by the manufacturer or the Supplier in an appropriate manner.
