Description

I. 1. The subject of the contract is the commissioning of advisory services and environmental supervision in the planning and implementation of maintenance and adaptation works in summer bat positions. 2. Substantive advice on the selection of a minimum of 55 to a maximum of 65-year-old bat sites of the following species: a small podkowiec, a large nightclub and a hatched nightclub in Małopolska, where cleaning works (e.g. guana cleaning, replacing or spreading the film) and adaptation and repair works are necessary. The works are planned to be carried out in the facilities in the period from December 2025 to March 2026. 3. Details of the scope of the subject-matter of this contract (including a detailed description of how the task is to be carried out) are set out in Annex 9 to the FTZ – Description of the subject-matter of the contract (OPZ). II. The tender in the proceedings shall be submitted, failing which it shall be invalid in electronic form, i.e. with a qualified electronic signature. III. 1. Documents to be submitted by the contractor together with the tender: 1) the completed Offer Form (Annex 1 to the SWZ); 2) a completed list of additional expert experience (Annex 1A to the FTZ) - if the Contractor wants to receive additional points in the Tender Evaluation Criterion; 3) power of attorney or other document confirming the power of attorney to represent the contractor – if the tender or the statements or documents attached to it are signed by a proxy. 4) power of attorney to represent all Contractors jointly applying for a contract, possibly a cooperation agreement, from which the power of attorney will result. 5) EAT (in accordance with the instructions in Annex 2 to the FTZ), appropriate as at the date of submission of tenders, in order to temporarily confirm that the Contractor is not subject to exclusion from the procedure pursuant to Art. 1 and Article 109 para. 1 point 4 of the PPL Act and meets the conditions for participation in the procedure set out in the FTZ. Where the Contractors jointly apply for a contract, the SEDZ shall be submitted by each of the Contractors jointly applying for the contract; 6) SEDZ concerning other entities referred to by the Contractor, in order to temporarily confirm, as at the date of submission of tenders, that there are no grounds for exclusion in respect of them pursuant to Art. 1 and Article 109 para. 1 point 4 of the Public Procurement Act and meeting the conditions for participation in the procedure specified in the FTZ, to the extent that the Contractor refers to their resources; 7) where the Contractor relies on the capacities or circumstances of the entities making resources available: evidence that the Contractor will have at his disposal the necessary resources of other entities when performing the contract, which constitutes, in particular, an obligation on those entities to place at the Contractor’s disposal the necessary resources for the performance of the contract or other subjective means of proof that the Contractor will have at his disposal the necessary resources of those entities when performing the contracts. (8) a declaration by the Contractor, as appropriate on the date of submission of the tenders, to confirm provisionally that the Contractor is not excluded from the procedure pursuant to Article 5k of Council Regulation (EU) No 833/2014 and Article 7(1)(b) and (c) of Regulation (EU) No 833/2014; 1 of the Act on special solutions for counteracting the support of aggression against Ukraine and for the protection of national security, in accordance with the model constituting Annex No. 8 to the FTZ. Where the Contractors jointly apply for a contract, the declaration shall be made by each of the Contractors jointly applying for the contract; 9) a statement by another entity on whose resources the Contractor relies, submitted for provisional confirmation, as at the date of submission of tenders, respectively, that that entity is not excluded from the procedure pursuant to Article 5k of Council Regulation (EU) No 833/2014 and Article 7(1)(b) of Regulation (EU) No 833/2014. 1 of the Act on special solutions for counteracting the support of aggression against Ukraine and for the protection of national security, in accordance with the model constituting Annex No 8 to the FTZ; 10) a statement by the contractors jointly applying for the award of the contract, which shows which supplies or services will be provided by individual contractors, in accordance with the model set out in Annex 7 to the FTZ (if applicable); III. The Contractor shall be excluded from the procurement procedure, subject to Art. 2 of the Public Procurement Act, pursuant to :Article 108(1) of the Public Procurement Act, Article 109(1) 1 point 4 of the PPL Act and on the basis of: Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine Ooh, ooh, ooh.
