PL-Krakow: medical practice services
1. The subject-matter of the contract is the provision of medical services in the area of preventive health care for police officers and employees of the District Police Headquarters in Proszowice and police officers and field staff of the Police Headquarters performing their duties on the territory of the District Police Headquarters in Proszowice.
2. The services will be provided in the city of Kraków.
3. The scope of the tests is defined in the occupational health service regulations and provides for:
(1) preventive and control tests of police officers,
(2) preliminary, periodic and inspection tests of police staff,
(3) health-epidemiological tests,
(4) medical examinations of drivers of emergency vehicles in the Police,
(5) preventive vaccination of police officers and workers (medical examination, vaccine, injection),
(6) qualification of people for preventive, rehabilitation and anti-stress tournaments,
(7) other benefits under occupational health regulations.
4. The expected quantities of medical treatment given in the table in the ‘Tender’ form, which is attached as Annex 2 to the ToR, are forecast values. These values are indicated only in order to select the most advantageous tender by the Contracting Authority and shall not constitute an obligation on the part of the Contracting Authority to the Contractor.
5. If a person is referred to specialised or laboratory tests not included in the price form, the contractor will be obliged to settle them at a price not higher than in the list of medical services of the medical unit concerned.
