PL-Łomża: gas oils
3.1. The subject matter of the contract is the successive supply of liquid fuels to the official means of transport, machinery and equipment used by the Contracting Authority in the form of:
(a) cash-free refuelling at a service station in the city of Łomża,
(b) deliveries to the container service station of the Waste Processing and Disposal Plant, near the village of Korytki Leśne and Czartoria, 18-413 City.
3.2. The service station shall have:
(a) a minimum of two stations for the simultaneous refuelling of unleaded petrol Pb 95 and diesel ON,
(b) cash and storage system with automatic service of fuel distributors,
(C) the station’s video-monitoring system equipped with a data archiving device.
The contracting authority shall allow the possibility of refuelling at other service stations of the Contractor if this is beneficial to the Contracting Authority for logistical, organisational and other reasons.
Liquid fuels offered should meet the quality requirements laid down in the Regulation of the Minister for the Economy of 9 December 2008 on quality requirements for liquid fuels (Journal of Laws 2008, No 221, item 1441).
3.3.Provisioned quantity that the Contracting Authority intends to order within a period of 12 months:

The quantities of liquid fuels indicated are estimates, determined on the basis of expected consumption, used to calculate the offer price. The final amount of fuel purchased will result from the performance of the contract until the end of the contract term, in accordance with the current needs of the Contracting Authority. Failure by the Contracting Authority to perform the contract in its entirety may not give rise to financial claims by the Contractor.
3.4.Names and codes used in the Common Procurement Vocabulary (CPV):

— 09134100 – diesel ON.
3.5. Fuelling will consist of successive (required) refuelling of vehicles into a full tank at the Contractor’s service station by an employee.
3.6. In the case of non-cash refuelling at a service station in the city of Łomża, the maximum distance between the Contractor’s service station and the premises of the Contracting Authority located in Akademicka Street 22, measured on the shortest safe route, only on paved roads where there are no restrictions on truck traffic may not exceed 3 km.
3.7. The supplier undertakes, for the duration of the contract, to maintain the price in accordance with the offer and the price of fuel may be changed only in the event of a change in the prices of the manufacturer (refinery) from which the Contractor is supplied. Each sale of fuel will be made subject to a fixed discount (the discount) provided by the Contractor in the tender form.
3.8. The Contractor shall clearly identify the time limit for refuelling and the quantity of fuel taken from the vehicle by drawing up a list of quantities, after the end of the week, in which the following identification information will be provided:
(a) the date on which the individual transaction was carried out,
(b) the road card number or the name of the fuel charger,
(C) the registration number of the vehicle or the name of the machinery, equipment,
(D) the type of fuel tanked,
(e) the amount of fuel tanked,
(F) the unit price of fuel in force at the service station on the day of refuelling,
(g) the driver’s signature.
3.9. Settlements for actual purchases of fuel by the Contracting Authority will be made after the end of the week on the basis of a VAT invoice, by transfer within 30 days of receipt by the Contracting Authority.
3.10. The information listed in point 3.6 will be attached to the VAT invoice and generated by the service station system without the possibility of intervention by the service station employee or driver.
3.11. In the case referred to in point. 3.1.b) The Contractor will be required to deliver and refuel gas oil at its own expense to the container service station of the Waste Processing and Disposal Facility – near the villages of Korytki Leśne and Czartoria, 18-414 City.
(a) deliveries will take place successively. The procurer will declare the quantity of gas oil procured on a case-by-case basis. The execution of the contract will take place within a maximum of two working days from the date of notification.
(b) the delivery of gas oil will be confirmed by an acceptance report drawn up on the basis of the metering list of the service station tank. The acceptance report must be signed by an employee of the Waste Processing and Disposal Facility.
(C) settlements for the supply of gas oil to the container service station of the Waste Processing and Disposal Plant following each delivery of the quantity of gas oil ordered, on the basis of a VAT invoice, by transfer within 30 days of its receipt by the Contracting Authority.
