1.The subject of the order is a service consisting in securing the needs of the army in portable shower containers along with their service: renting, setting, cleaning and technical service in facilities administered by the 16th Military Branch of Gospo-donczy in Drawsko Pomorskie. The order covers the area of responsibility 16 WOG in the following locations Grupa Zabezpieczenia Drawsko Pomorskie Grupa Zabezpieczenia Złocieniec Grupa Zabezpieczenia Wałcz Grupa Zabezpieczenia Wałcz/Nadarzyce 2. The Ordering Party is entitled to change the number of days and the number of devices with the proviso that the value of the entire contract may not be increased. 3. A detailed description of the subject of the contract is included in the annexes to the FTZ, in particular in: a) description of the subject of the contract b) draft contract 4. The performance of activities related to the provision of the service consists in comprehensive daily service of sanitary equipment, i.e. emptying drainless tanks, removal and disposal of bath effluent from container baths, ongoing replenishment of washing agents or, if necessary, disinfectants in soap dispensers, placed above the washbasins located in each type of shower containers. Continuous maintenance of adequate hygienic and cleanliness conditions, consisting in the performance by persons directly providing work on the above-mentioned activities in the manner specified in Article 22(1) of the Labour Code of 26 June 1974, in connection with the above, the Contracting Authority requires the employment by the Contractor and/or Subcontractors of all the above-mentioned employees performing the service for the entire duration of the contract on the basis of an employment contract. 5. How to verify the employment of these persons ; During the execution of the Order, at each request of the Ordering Party within the time limit specified in this request, the Contractor shall submit to the Ordering Party one of the following evidence in order to confirm the fulfilment of the requirement of employment on the basis of an employment contract by the Contractor or Subcontractor of the persons performing the persons indicated in paragraph. 4 actions during the execution of the Order: 1) Declaration of the Contractor or Subcontractor on the employment on the basis of an employment contract of persons performing activities covered by the Employer's request. That statement shall include, in particular: a precise determination of the entity submitting the declaration, the date of submission of the declaration, an indication that the activities covered by the request are performed by persons employed on the basis of an employment contract, together with an indication of the number of these persons, the type of employment contract, the scope of duties and the length of time worked, and a signature of the person authorised to submit the declaration on behalf of the Contractor or Subcontractor; 2) An anonymised copy of the contract(s) of employment of persons performing the activities covered by the above-mentioned statement of the Contractor or Subcontractor (together with the document regulating the scope of duties, if it has been drawn up) certified as being in conformity with the original by the Contractor or Subcontractor, respectively. An anonymised copy of the contract(s) should include: the type of activities performed (position), the date of conclusion of the contract, the type of contract and the length of time the employee is employed; 3) An anonymized copy of proof confirming the employer's notification of employees performing activities for the Employer to social and health insurance.
