Description

The subject of the contract is the Supply of equipment for the treatment of chronic coronary artery occlusions (CTO) in the Laboratory of Surgical Radiology Hemodynamics and Electrocardiology of the Świętokrzyskie Centre of Cardiology of the Provincial Complex Hospital in Kielce A detailed description of the subject of the contract and the quantity of the ordered assortment is set out in Appendix No. 2 to the SWZ - assortment and price form for Packages 1 to 7, Appendix No. 2.1 to the SWZ _ Summary of leased equipment for the project. Package 6Information on the rules for delivery, guarantees and remuneration is contained in Annex 3 to the FTZ – the draft provisions of the public procurement contract on the basis of a deposit for: Package 1, 2, 3, 4 and 5, Annex 3a to the FTZ – draft provisions of the public procurement contract for successive deliveries concerning Package 6 and 7 and Annex 3b to the FTZ – draft provisions of the lease agreement for Package 6. The contracting authority reserves the right to modify the draft provisions of the public procurement contract – Annex No 3 to the FTZ and Annex No 3a to the FTZ, if a single economic operator is awarded a public contract for more than one task of this procedure.
