Description

1. The subject of the contract is the provision of: 1) comprehensive medical services in the field of occupational medicine for Bank Gospodarstwa Krajowego employees and specialized medical services for Bank Gospodarstwa Krajowego employees and their families and relatives in the form of medical packages; 2) Provision of occupational health services, in accordance with the provisions on the occupational health service, in particular on the basis of the Act of 27 June 1997 on the occupational health service . A detailed description of the subject of the order, conditions for the execution of the order, payment, etc. is specified in the Description of the subject of the Order (hereinafter: ‘OPZ’), which is Annex 1 to the draft provisions of the Agreement (hereinafter: ‘PPU’), which form Annex 2 to the FTZ.
