236/PN/2024 - Supply of medicinal products for UCK Items 241-260
1. The subject of the contract is the supply of medicinal products for UCK in the assortment, the estimated quantity and the requirements set out in Annex 3 to the SWZ. 2. The contracting authority shall allow partial tenders to be submitted. Partial tenders are understood as individual items set out in Annex 3 to the ToR. The Contractor may submit a tender for one or more items. 3. As part of the items marked in the assortment and price form with the symbol (**) in Annex 3 to the SPC, the Ordering Party requires that the offered preparation be included in the catalogue of reimbursed medicines used in drug programs or in the catalogue of reimbursed medicines used in chemotherapy. 4. The ordering party specified the method of packaging in the assortment and price form. In items where there is no such description, the Contractor may offer any method of packaging in accordance with the quantity required by the Ordering Party specified in the item. The contractor must estimate the proposed packaging method with an appropriate calculation. 5. Deliveries will be made successively, in quantity and assortment, in accordance with partial orders of the Ordering Party within 2 days from the date of placing the order. The Contractor will be obliged to deliver the ordered assortment at his own expense to the Hospital Pharmacy. 6. The Contracting Authority requires that the prices of medicines offered are not higher than the official prices referred to in Art. 1 and 2 of the Act of 12 May 2011 on the reimbursement of medicines for foodstuffs intended for particular nutritional uses and medical devices (Journal of Laws 11.122.696), applicable on the day of submission of tenders, if applicable. 7. The offered medicinal products must comply with the Pharmaceutical Law Act of 6 September 2001 (Journal of Laws No. Journal of Laws 2022, item 2301), if applicable. The offered medicinal products must be authorised for marketing in Poland on the terms set out in Article 3 or 4a of the Pharmaceutical Law Act. 8. The contracting authority shall allow an equivalent tender to be submitted in respect of items 26, 30, 260, 261, 262. The Ordering Party informs that any proper names used in the description of the subject of the contract specify the required standard of the ordered assortment. The Contracting Authority allows the submission of tenders with equivalent quality parameters, having the same chemical composition in terms of basic substances. Proof of compliance with the equivalence condition shall rest with the Contractor.
