Description

1. The subject of the contract is the successive supply of medical equipment – 50 insulin pumps for UCK with the technical parameters set out in Annex 4 to the FTZ, hereinafter referred to as ‘equipment’, together with the installation and training of the Contracting Authority’s employees. 2. The above-mentioned medical equipment must meet the limit parameters set out in Annex 4 to the FTZ. Failure to meet at least one of the limit parameters set out in Annexes 4 to the FTZ will result in the rejection of the tender. 3. The equipment will be supplied with the equipment necessary for its proper functioning. 4. The equipment will be delivered to the place indicated by the Ordering Party in Gdańsk, at ul. Smoluchowskiego 17 at the expense of the Contractor. 5. The Contractor shall train the Employer's employees on the basics of the operation of medical equipment within the time limit specified in the draft contract. 6. The contractor will be obliged to ensure that the medical equipment supplied is brand new, complete, of a high standard in terms of both quality and functionality, as well as free from material, construction and legal defects, approved for use in public health care facilities. 7. The Contractor will be obliged to provide, within the time limit for the first delivery of medical equipment, copies of documents certified as being in conformity with the original, from which it appears that the offered assortment has been approved for marketing and use in the Republic of Poland. 8.The offered medical equipment must be brand new manufactured no earlier than 2024.
