Description

The subject of the contract is the supply of medical equipment for the operating room of the cardiac surgery sub-unit of the Multi-specialist Provincial Hospital in Gorzów Wielkopolski Sp. z o.o. A detailed description of the subject of the contract and estimated quantities are included in: a.Annex 2 to the SWZ – Price form, b. Annex 2.1 to the SWZ – Summary of technical parameters, c. Annex 3 to the SWZ – draft contract – constituting annexes to the SWZ respectively.
The offered medical device should be allowed to be placed on the market in accordance with the applicable regulations, i.e. in accordance with the Act of 7 April 2022 on medical devices and its implementing acts. The document authorising a medical device to be placed on the market shall mean, in particular, the CE declaration of conformity.
