2. Nazwa międzynarodowa, dawka oraz ilości przedmiotu zamówienia określa formularz asortymentowo-cenowy nr 1.1 stanowiący załącznik do formularza ofertowego.
3. Termin ważności oferowanego produktu nie może być krótszy niż 12 miesięcy liczony od daty dostawy.
