Description

The subject-matter of the contract is a group life insurance service for employees, spouses and adult children of employees of Rybnik Municipal Council. The insurance cover covers: 1. Insurance against the death of the insured person and, in addition: (1) life insurance of the insured following an accident, (2) death insurance following an accident at work, (3) death insurance of an insured person following a motor accident, (4) death insurance of an insured person following a traffic accident, (5) death insurance of the insured following a heart attack or stroke, 2 insurance against the death of the spouse and, in addition, insurance against the death of the spouse following an accident, 3. insurance against death of parents or parent-in-law, 4. death insurance, 5. birth insurance, 6. stillbirth insurance, 7. orphanage insurance, 8. permanent injury insurance Insured following an accident, 9. insurance against permanent injury to health Insured following a heart or stroke, 10. severe illness insurance, 11. Insured surgical surgery, 12. Specialised treatment insurance, 13. Insured treatment insurance in hospital for sickness (including IOM stay and convalescence) and additionally insurance for hospitalised treatment for heart inflict or stroke, 14. Insured hospital treatment insurance for personal injuries as a result of an accident (including stay at the OIOM and convalescence) and, in addition: (1) Insured hospital injury insurance as a result of an accident at work, (2) Insured treatment insurance for personal injuries following a traffic accident, (3) Insured treatment insurance in hospital following a traffic accident. The number of employees employed at the City Hall as at 6 November was 472, but the Contracting Authority reserves a change in the number of insured persons in keeping with the premium rate indicated in the tender. The detailed scope of the contract shall specify the ToR and its annexes.
