Description

1. Przedmiotem zamówienia jest świadczenie usług medycznych w zakresie medycyny pracy w szczególności badań profilaktycznych, wstępnych, okresowych i kontrolnych dla pracowników oraz kandydatów na pracowników kierowanych przez Pocztę Polską S.A. Centrum Kadr i Szkoleń Dział Zamiejscowy w Olsztynie oraz inne komórki kadrowe Poczty Polskiej S.A. – zgodnie z wzorem umowy stanowiącym Załącznik nr 3 do SWZ.
2. Szczegółowy zakres usług znajduje się w opisie przedmiotu zamówienia stanowiącym Załącznik nr 4 do SWZ, w Załączniku nr 1.1, 1.2, 1.3, 1.4, 1.5 i 1.6 do SWZ – Formularzach rzeczowo-cenowych oraz Załączniku nr 3 do SWZ – Wzorze umowy.
