Description

1. The subject-matter of the contract is the supply of an oxygenator used for the life-saving procedure in the Clinica of Aesthesia and Intensive Therapy, a detailed description of which is set out in Annex 1 to the ToR – ‘Tender Form – Price Form’.
2. The contracting authority does not allow partial tenders to be submitted. The subject matter of the contract is one task.
3. The contracting authority shall not accept equivalent solutions.
4. The period of validity of the delivered product must be – at least 24 months from the date of delivery of the goods to the Contracting Authority’s premises.
5. The contracting authority shall require that the subject-matter of the contract offered meets the following conditions (if specific to its specificities):
(a) packaged separately (individually),
(b) each individual packaging shall indicate that it is a sterile single-use or biologically pure single-use product,
(C)each individual package must contain a description in Polish,
(D)each individual package must bear a durable and legible date
