1. The subject of the contract is the delivery of three road ambulances for the needs of the medical rescue team of the Bielski Ambulance Service: • One Type C container ambulance • Two Type A2 electric ambulances with multi-purpose stretcher, cardiac chair 2. A detailed description of the subject of the contract is set out in Annex 1 to the SWZ. 1. List of declarations made by the Contractor to confirm in advance that he is not excluded from participation in the procedure, together with his tender: 1) Together with the tender, the Contractor shall submit a declaration, valid as at the date of submission of tenders, that the Contractor is not subject to exclusion, submitted in the form of a European Single Procurement Document (ESPD). – Annex 3 to the SWZ (the statement constitutes evidence confirming the absence of grounds for exclusion of the Contractor as at the date of submission of tenders, temporarily replacing the subjective evidence required by the Contracting Authority, indicated in the SWZ) and a statement regarding the lack of grounds for exclusion on the basis of the conditions set out in Chapter IX, point. 1.3 and points. 6 – in accordance with Annex 4 to the SWZ. (2) The ESPD should be drawn up in accordance with the model of the standard form set out in Commission Implementing Regulation (EU) 2016/7 of 5 January 2016 establishing a standard form for the European Single Procurement Document (OJ Urz. EU L 3, 06.01.2016, p. 16) to the extent that it meets all the requirements set out in the SWZ. The Contracting Authority may require the Contractors to explain the content of the means of proof in question. 3.
