1. The subject of the contract is ‘Supply of reagents for biochemical and allergological tests together with the lease of analysers for the Laboratory Diagnostics Department ‘Pro-Medica’ in Ełk Sp. z o.o.’, No 2076/2025. 2. A detailed description of the subject-matter of the order (OPZ) and the manner of execution of the order contains the Description of the subject-matter of the Order constituting Annex No. 4 to the FTZ and the Assortment and Price Form constituting Annex No. 3 to the FTZ. 3. In addition, the subject of the contract is: 1) (Package 1) Lease of one analyzer for a period of 25 months, i.e. from 01.09.2025r. to 30.09.2027r. 2) (Package 2) Lease of two analyzers for biochemical research for a period of 25 months, i.e. from 01.09.2025 to 30.09.2027. 3) (Package 2) making available as part of the price for the subject of the order one refrigerator with a website for storing reagents for the entire duration of the contract. 4. The Ordering Party allows the submission of partial tenders (Package 1-2). 5. The scope of the Contractor’s responsibilities and the manner in which the contract is to be performed are set out in detail in Annex 5 to the FTZ – Draft provisions of the contract. 6. The subject-matter of the contract must meet the requirements laid down in the Medical Devices Act of 7 April 2022 (Journal of Laws 2022, No. Journal of Laws 2024, item 1620), must have documents authorising the placing on the market and use in Poland, in particular be marked with the CE mark indicating compliance with the requirements of the European Union.
