Description

1. The subject of the contract is the supply of reagents, control and consumable materials, control blood together with the lease of analysers and tests specified in parts for the Fryderyk Chopin University Clinical Hospital in Rzeszów. 2. A detailed description of the objects of the order, the quantities of deliveries and the requirements are included in: a) Price and offer forms for Lot No. 1÷ 8 (Annexes No. 1 to the SWZ); b) Price and offer forms for the lease of the analyzer for Parts No. 1÷ 4.8 (Annexes No. 2 to the SWZ); Price and offer forms – summary statement for Lot No 1÷ 4.8 (Annexes No 3 to the SWZ); Compilation of limit values for biochemical analyser and reagents for Part No 1 (Annex 4 to the SWZ); e) Maintenance conditions of the analyzer for biochemical tests for Part No. 1 (Annex No. 5 to the SWZ); f) Compilation of the evaluated parameters of the analyzer for biochemical tests for Part No. 1 (Annex No. 6 to the SWZ), g) Compilation of the required parameters of the 5 Diff haematological analyzer for Part No. 2 (Annex No. 4 to the SWZ); h) Compilation of the assessed parameters of the 5 Diff haematological analyzer for Part 2 (Annex 5 to the SWZ); i) Compilation of the parameters required of the analyser for coagulology tests for Part No 3 (Annex No 4 to the SWZ); j) Compilation of the evaluated parameters of the analyzer for coagulology tests for Part No. 3 (Annex No. 5 to the SWZ); Compilation of the required parameters of reagents, calibrators, control and consumable materials for coagulology tests for Part 3 (Annex 6 to the SWZ); Compilation of the parameters required for an automatic Gram dyeing analyser for Part 4 (Annex 4 to the SWZ); m) Compilation of the required parameters of the 5 Diff haematological analyzer for Part No. 8 (Annex No. 4 to the SWZ); which are an integral part of the FTZ. 4. The scope of the contract includes the delivery of the subject-matter of the contract in the quantities specified in the Price and Offer Forms for the period of: in Parts 1-6 and 8 - 36 months from the date of conclusion of the contract; - in Part 7 - 24 months from the date of conclusion of the contract. 5. The contracting authority does not require and does not allow the possibility of submitting a variant offer referred to in Article 92 of the Public Procurement Act. 6. The Contracting Authority does not allow the submission of equivalent tenders for Parts 1, 4, 5, 6, 7. 7.The Ordering Party allows solutions equivalent to those described in Part No. 2 - Annex No. 5 to the FTZ, in Part No. 3 - Annex No. 6 to the FTZ, in Part No. 8 - Annex No. 4 to the FTZ. Whenever the description of the subject of the contract refers to standards, technical assessments, technical specifications and technical reference systems referred to in Art. 1 point 2 and paragraph. 3 PPL, it is assumed that such indications are accompanied by the words "or equivalent". The Contractor who refers in the tender to solutions equivalent to those described by the Employer is obliged to demonstrate in the tender, in particular by the means of evidence referred to in Articles 104-107 of the Public Procurement Law, that the solutions proposed by him meet the requirements specified by the Employer in the description of the subject of the contract to an equivalent degree. 8. DATE OF ORDER PERFORMANCE: 1. Successive delivery of order items by: -Parts 1-6 and 8 for a period of 36 months from the date of conclusion of the contract, - Part 7 for a period of 24 months from the date of conclusion of the contract, (does not apply to the lease of equipment). 2. The contracting authority allows the possibility of changing the term of the contract in the event of failure to complete the entire subject of the contract during the contract period as a result of the performance of lower orders. Amendment of the contract requires an annex, except that the Ordering Party unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the period for which it was concluded (does not apply to lease).
