Description

1. The subject of the order is the supply of accessories for endoscopic treatments. The subject of the contract was divided into 12 tasks (including Task 3 of the Commission) – in accordance with Annex 1 to the FTZ ‘Assortment and price form’. 2.A detailed description of the subject of the contract, including the required parameters, size, feature, i.e., quantity is specified in the ASORTIMENT AND PRICE FORM, constituting Annex No. 1 to the FTZ. The parameters described in Annex 1 to the FTZ, as required parameters, are the limit requirements, the non-fulfilment of which will result in the rejection of the offer. 3. Detailed conditions and rules for the performance of the subject-matter of the contract are laid down in the provisions of the Model Contract – Annex 5 to the FTZ ‘Model Contract’. 4. The Contractor shall deliver the subject of the order to the APTEKI SZPITALA Warehouse. 5. Deadline for the execution of the contract for 12 lots (12 Tasks): 18 months from the date of conclusion of the contract. 6. Delivery date according to current needs, each time within a maximum of 3 working days from the date of placing the order.
