Description

1. The subject of the contract is the supply of medical equipment financed under the investment entitled: ‘Equipment with medical equipment and apparatus of the Hospital Emergency Department of Pałucki Centrum Zdrowia Sp. z o.o. in Żnin’. 2. The contract is implemented under contract No DOI/FM/SMPL/102/MDSOR/2023/1825/404 for the award of a special-purpose grant to finance the implementation of the investment entitled: "Equipment with medical equipment and apparatus of the Hospital Emergency Department of Pałucki Centrum Zdrowia Sp. z o.o. in Żnin" 3. A detailed description of the subject of the contract is contained in the annexes to the SWZ, including: Annex 2.1 and 2.2 to the SWZ – Summary of technical conditions and parameters and Annex 3 to the SWZ – Scope of adaptation works. 4. The parameters described in Annex No. 2.1 and 2.2 to the SWZ as a Summary of conditions and technical parameters constitute a detailed description of the subject of the contract, the non-fulfilment of which will result in the rejection of the tender as inconsistent with the terms of the contract pursuant to Art. 1 point 5 of the Public Procurement Act. No entry in the Parameters offered section will be treated as non-fulfilment of the parameter resulting in the rejection of the offer. 5. The order is divided into 2 packages. The Contracting Authority shall allow partial tenders to be submitted divided into packages: Package 1 – Radiological equipment for diagnostic laboratories (computer tomograph 128 layers, digital x-ray machine, x-ray machine with C-arm) including installation and adaptation of rooms. Package 2 – Ultrasound 6. As part of Package 1, the Ordering Party requires the submission of a full offer, i.e. the offer must cover the entire subject of the contract in terms of assortment and quantity.
