Description

1. The subject of the contract is the delivery of medical devices to the URS along with the rental of an image sensor to the URS to the seat of the Ordering Party (31-826 Kraków, pers. Golden Autumn 1), in accordance with the description and requirements contained in Appendix No. 1A and 1B to the nin. Specifications. 2. The validity/guarantee period of the subject of the contract may not be less than 12 months from the date of delivery. 3. Requirements for medical devices for URS: a) The Contractor undertakes to conduct, at the request of the Contracting Authority, training of the Contracting Authority's staff at its premises. Golden Autumn 1, 31-826 Kraków together with the first delivery of goods to the Hospital Pharmacy within the period agreed in writing with the Ordering Party, but not later than within 30 days from the date of conclusion of the contract. 3 letter a with the signatures of trained users confirming their actual presence to the Hospital Pharmacy, within 5 working days from the date of the training. 4. Requirements for an image sensor for URS: a) The Contractor is obliged on the day of delivery of the subject of the contract to train the Employer's employees from the delivered equipment at the Employer's headquarters (31-826 Kraków, pers. Golden Autumn 1) or the provision of such training by a third party at its own expense, which will be confirmed by the issue of personal certificates. b) The list of technical and operational limit parameters for the image sensor to the URS is contained in Annex 1B to the SWZ. 5.
