Description

1. The subject of the contract is the purchase, supply, assembly and commissioning of medical equipment and apparatus for the Cardinal Stefan Wyszyński Provincial Hospital in Łomża, in the quantity and with the parameters required in Annex 4 to the FTZ, together with full warranty service and training of the Contracting Authority’s staff, under the conditions set out in Annex 4 to the FTZ. The contract is implemented as part of the ‘Development of the infrastructure for diagnostics and oncological treatment of the Kard Provincial Hospital. Stefan Wyszyński in Łomża’ implemented under the National Recovery and Resilience Plan: Component D ‘Efficiency, accessibility and quality of the health system’ Investment D1.1.1 ‘Development and modernisation of the infrastructure of highly specialised care centres and other medical entities’ – concerns Task 4: “Purchase of specialised medical devices”. 2. The conditions for the performance of the contract are set out in the model contract constituting Annex 5 to the FTZ. 3. The order is divided into 2 parts, called Packages. The contracting authority shall allow partial tenders to be submitted. The offer for the Package is accepted as a partial offer. The Contractor may submit an offer for one or more Packages. The Ordering Party shall not limit the number of Packages for which the Contractor may submit an offer and shall not limit the number of Packages for which the Ordering Party may award the Contractor's contract. The Package is indivisible; when submitting an offer for the Package, the Contractor is obliged to offer and make a valuation of each item of the Package.
