Description

Przedmiotem zamówienia jest dostawa implantów ortopedycznych, naczyniowych, sprzętu jednorazowego do operacji naczyniowych - DEPOZYT do Szpitala Praskiego p.w. Przemienienia Pańskiego Sp. z o. o. w Warszawi. Wykonawca, według własnego uznania, może złożyć ofertę na jedną bądź wszystkie części przedmiotu zamówienia.
Szczegółowy opis przedmiotu zamówienia określa Formularz asortymentowo-cenowy, stanowiący Załącznik nr 2 do SWZ (2.1-2.7 w zależności od części zamówienia),
Wykonawca może zaoferować wyłącznie wyroby medyczne, które zostały dopuszczone do obrotu i używania zgodnie z wymogami ustawy z dnia 20 maja 2010 r. o wyrobach medycznych (Dz. U. z 2017 r., poz. 211 z późn.zm.) i przepisami wykonawczymi do tej ustawy, Zamawiający zastrzega wgląd do aktualnych dokumentów na każde żądanie Zamawiającego na każdym etapie realizacji umowy.
