Description

1. The subject-matter of the contract is the supply of USG devices and gastroscopes, hereinafter referred to as medical devices. The detailed requirements concerning the minimum technical and performance parameters are set out in Annexes 1.1-1.9 to the ToR. The medical devices offered must be new, manufactured after 1 January 2016. Reconditioned devices and demonstrators may not be offered. Medical devices must be free from any physical (constructional) and legal defects.
2. The medical devices offered must have the required certificates and markings provided for in the Medical Devices Act attesting to the required authorisation for use and marketing in Poland, i.e. the CE certificate and the declaration of conformity in accordance with the Regulation of the Minister of Health on essential requirements and conformity assessment procedures for medical devices.
