The subject-matter of the contract is the insurance service for patients in respect of medical events for the Provincial Self-Self-Self-Self-House in Gorzów Wlkp.
A detailed description of the subject matter of the contract.
Insurance coverage – insurance for patients in respect of medical events (in accordance with Article 25. 1 point 2. Act of 15.4.2011 on therapeutic activities – Journal Journal of Laws No 112, item 654) which took place during the period of insurance cover within the meaning of the Patients’ Rights Act and the Patients’ Rights Ombudsman (Act of 6 November 2008 – Journal Journal of Laws 2009, No 52, item 417 and subsequent amendments, including the Act of 28.4.2011 amending the Patients’ Rights Act and the Patients’ Rights Ombudsman and the Act on Compulsory Insurance, the Insurance Guarantee Fund and the Polish Motor Insurers’ Bureau – Dz. U. of 2011 No 113, item 660).
Sum insured: in accordance with the Act of 28.4.2011 amending the Patient Rights Act and the Patients’ Rights Ombudsman and the Compulsory Insurance Act, the Insurance Guarantee Fund and the Polish Motor Insurers’ Offices (Journal of Laws Journal of Laws No 113, item 660):
PLN 1 200 000,00 in the annual insurance period for all medical events covered by insurance cover, except that in the case of:
(1) Infection, bodily injury or health disorder of the patient – is: PLN 100 000,00,
2) the death of the patient – is: PLN 300 000,00 per patient.
