Description

Staffordshire County Council, in conjunction with its NHS Clinical Commissioning Group Partners, is carrying out this tendering exercise to appoint a suitably qualified organisation to provide a high quality, outcome focussed support service for Looked After Children and Care Leavers.
These services will provide support at the earliest available opportunity, and better link the process of statutory health assessments with the subsequent planning of additional health care and support services effectively in a cohesive and timely manner. We want to streamline the way services that support some of our most vulnerable children are provided, and work effectively to help young people in care live healthy, independent lives and be able to take responsibility for their own good health.
