Description

The public procurement procedure is conducted in an open tender procedure pursuant to Article 132 of the Public Procurement Act. The subject of the contract is the supply of medical devices for hemodynamics according to the following parts: Task 1 System to temporarily support the work of the left ventricle of the heart, Task 2 Cryoablation kit, Task 3 Pulsed electric field ablation kit (PFA), Task 4 Cable for intraoperative measurements. The detailed description of the subject-matter of the contract for the above-mentioned lots is shown and described in the ‘Price Form’ (Annex 2 to the SWZ), the ‘Parameter Sheets’ (Annex 2a to the SWZ) and in the model contracts (Annex 3, Annex 3a to the SWZ), which form an integral part of the tender specifications. The contractual conditions for the performance of the public contract in question include model contracts, which form an integral part of the TMS. If there are grounds for entrusting the processing of personal data, the Contractor undertakes to sign a contract for entrusting the processing of personal data (in accordance with Annex 11 to the SWZ).
