1. The subject of the contract is: DELIVERY OF SINGLE-USE STAPLERS, LOADS AND J.U. ENDS TO THE HARMONIC KNIFE for the Provincial Specialist Hospital in Częstochowa. 2. The description of the subject of the order was included in the assortment and price forms, constituting Annex 2 to the FTZ. The other conditions for the performance of the contract are set out in the draft provisions of the contract (PPU), constituting Annex 6 to the FTZ. 3.
