1. The subject-matter of the contract is the supply of medical equipment of Głogowski Szpital Powiatowy sp. z o.o., hereinafter also referred to as ‘the assortment’. 2. Place of delivery and assembly: Głogowski Szpital Powiatowy sp. z o.o., ul. Kościuszki 15, 67-200 Głogów. 3. The offered assortment must be brand new, manufactured in 2024. 4. In order to confirm the requirements in relation to the offered assortment, the Contractor will attach to the offer information materials: catalogues, photos, folders, leaflets, descriptions, etc. 5. The Ordering Party does not allow copying of publicly available materials - it is required to present materials regarding the actually offered assortment in order to verify whether it meets the requirements of the Ordering Party. 6. If any standards/certificates/athletes/classes/standards are indicated in the detailed description of the contract, the Contractor shall demonstrate that the range offered meets the Contractor's requirements in this respect. 7. The contract consists of four lots and includes the supply and assembly of medical equipment Part No 1 – supply and assembly of the operating table; Part 2 – supply and installation of 6 defibrillators; Part 3 – delivery and assembly of the treatment table; Part 4 – Supply and assembly of an anaesthetic apparatus; the description of the subject-matter of the contract is set out in Annexes 2.1 to 2.4 to the SWZ (technical specifications for equipment). 8. The conditions for the performance of the contract are set out in the draft contract, which constitutes Annex 3 to the SWZ. 9. Main subject of the contract according to the Common Procurement Vocabulary (CPV):33.10.00.00-1 Medical devices. 10. The offered assortment must meet the requirements of the Ordering Party given in the detailed description and specifications and fully meet the functional requirements, in accordance with their intended purpose. 11. The subject of the contract offered must meet the conditions required by law for its application. They must have documents allowing them to be used in healthcare facilities in the Republic of Poland, in accordance with the requirements set out in the Act of 7 April 2022 on medical devices and its implementing provisions, i.e. a declaration of compliance with the essential requirements of the CE and, if required by separate provisions, a certificate of a notified body that participated in the assessment of a medical device, as set out in the Selected 12 Act on medical devices. The Contractor undertakes, as part of the future contractual performance, to deliver and assemble the subject of the contract in the place indicated by the Ordering Party in sec. 2 of this chapter, at your own expense and risk.
