Description

3. Description of the subject of the contract
3.1. The contract concerns:
Supply of disposable medical equipment:
Task 1 – Single-use ends for Shaver for the Orthopaedia and Traumatology Bloc
Task 2 – Terminals for Vapr III for the Orthopaedia and Traumatology Bloc
Task 3 – System of separation of platelet growth factors for the Ortopedia Bloc and the Movement Organ Traumatology
Task 4 – Terminals for pulse rinsing kits (Lavage) for the Ortopedia and Traumatology Bloc of the Movement Organ
Task 5 – Post-operational retransfusion and suction retransfusion kit for the Orthopaedia and Traumatology Bloc of the Movement Organ
A detailed description of the subject matter of the contract is set out in Annex 2b, Annex 4 and Annex 5 to the ToR.
3.3. The Contracting Authority’s requirements for the subject matter of the contract:
(1) the product offered must be certified, certified and registered in accordance with
with legal provisions requiring attestations, certificates and registration,
(2) the device offered must be placed on the market and put into service in accordance with
with the requirements of the law
(3) the product offered must comply with the standards or technical specifications applicable to the device;
In the event that the medical device offered does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor shall be required to carry out, at his own expense and on his own basis, all the necessary steps to comply with all the legal requirements relating to the placing on the market and use of the device in Poland. The contractor must do so within the time limits laid down by the applicable rules and provide the Contracting Authority with the relevant documents confirming that such activities have been carried out within these time limits.
3.4. In order to confirm that the supplies offered meet the requirements laid down by the Contracting Authority, the Contractor must submit together with the tender:
1 For each Task:
Description of the subject of the contract in accordance with the model set out in Annex 4 to the ToR
— in the form of an original or a copy certified as a true copy by the Contractor;
In the case of contractors jointly applying for a contract as referred to in Article 23 of the Public Procurement Act, these documents shall be submitted by their representative;
2 For the Tasks: 3 and 5:
A list of technical parameters unequivocally confirming the conformity of the parameters of the tendered subject of the contract with those specified in the ToR, drawn up in accordance with the model set out in Annex 5 to the ToR
— in the form of an original or a copy certified as a true copy by the Contractor;
In the case of contractors jointly applying for a contract as referred to in Article 23 of the Public Procurement Act, these documents shall be submitted by their representative;
