1.The subject of the order is: Delivery of medical equipment. 2. A detailed description and scope of the subject of the contract is set out in Annex 2 to the Terms of Contract (SWZ). 3. The Contractor is obliged to carry out the delivery of the subject of the contract. 4. The Ordering Party divided the order into individual parts and allows the possibility of submitting partial offers for any number of Packages - Packages: No 1 – No 4 5. The subject-matter of the contract must meet the requirements set out by the Ordering Party in the Terms and Conditions of the contract, in particular the conditions set out in the description of the subject-matter of the contract contained in Annex No 2 – Technical parameters to the FTZ. Failure to meet even one of the limit conditions specified in the above-mentioned Annexes will result in the rejection of the tender from the procedure. 6. Implementation of the task under contract No FM-STI.01.PEDIATRIA.2025-W-29263/305/31 for the award in 2025 of a specific grant for the implementation of the task consisting in supporting the development of investment solutions for the provision of healthcare services in the field of co-financing the purchase of medical equipment for the diagnosis and treatment of paediatric patients 8. Whenever the brand or origin of a product or appliance is indicated in the documentation, it must be assumed that the words ‘or equivalent’ are followed by each name, i.e. that the embedded materials, appliances, etc. will have (characterised by) all the parameters not inferior to those described in this documentation for the item in question. 9. The possible use of proper terms and names in the documentation is only illustrative and serves to determine the class and quality of materials. If the description of the subject-matter of the contract includes indications of trademarks, patents or origin, source or specific process that characterises the products or services provided by a particular Contractor, if this could lead to the preference or elimination of certain Contractors or products, unless this is justified by the specific nature of the subject-matter of the contract and the Contracting Authority cannot describe the subject-matter of the contract with sufficiently precise terms, and such indication is accompanied by the words ‘or equivalent’. The Contractor may offer equivalent items. The customer requires that the materials used, if different, have quality and technical parameters not inferior to those specified in the subject of the contract. It shall be for the Contractor to demonstrate the equivalence of the item offered. 10. The Ordering Party undertakes to implement the subject of the contract in the amount of a minimum of 100% of the gross value of the contract. 11.Deadline for performance of the contract: within 7 days from the date of conclusion of the contract.
