1. The subject of the contract is the supply of a two-plane angiograph with equipment and gas columns as part of the investment task ‘Extension of the main building of the Hospital for the neurosurgery and cardiac and neurological rehabilitation department as part of investment task No 91669’, together with the adaptation of the installation and system in terms of the equipment supplied and assembly divided into 2 parts: Task 1 – Angiograf with equipment Task 2 – Gas columns 2. The subject-matter of the contract includes the supply together with control systems and software, unloading, adaptation of the installation and assembly system with regard to the supplied equipment, as well as installation, training of personnel in the use and commissioning of the following requiring incorporation and permanent connections: 1 double-plane angiograph, 1 single-station column and 4 double-station columns, in accordance with the assortment and price forms constituting Annexes 3.1 to 3.2 to the SWZ and a detailed description of the subject-matter of the contract, the table of technical and operational conditions constituting Annexes 1.1 to 1.2 to the SWZ and the technical documentation constituting Annexes 14.1 to 14.20 to the SWZ.
