Description

1. The subject of the contract is delivery to the Provincial Hospital in Bielsko-Biała at Al. Home Army 101: 1) equipment for electrophysiological tests and ablation, by depositing with the Ordering Party, together with the lease of the system for electrophysiological tests and ablation and the system for three-dimensional cardiac mapping – Part 1, 2) equipment for cryoablation of pulmonary veins and point cryoablation – Part 2, according to the quantitative and generic list specified in Annex No 2 to the tender specifications, hereinafter referred to as ‘the above’, meeting the quality parameters specified in Annex No 2 to the above, under the conditions specified in the draft provisions of the contract constituting for Part 1 – Annex No 4a to the above, for Part 2 – Annex No 4b to the above. 2. The offered goods must be new and unused, with the exception of the leased item referred to in Part 1 and the loan item referred to in Part 2. 3. Delivery of the subject of the contract together with transport and unloading will take place at the expense and risk of the Contractor. 4. Applicable to Part 2 – During the period of performance of the contract, the Contractor is obliged, as part of the price of the subject of the contract in the scope of Part 2, to lend, at any request of the Ordering Party, a cryoconsole with a cooling agent, under the conditions specified in the draft provisions of the contract constituting Annex 4b to the Agreement.
