Description

The subject of the contract is the preparation and daily delivery of meals for hospitalized patients. The contract covers the delivery of the subject of the contract to the indicated reception zones specified in the Description of the subject of the contract and the distribution of meals located in hospital buildings: at ul. Kościuszki 29, ul. Zygmunt Stary 20, ul. Radio 2, ul. Kozielska 8, Detailed description of the requirements of the Ordering Party regarding the preparation and delivery of meals, is presented in Appendix No. 3 to the SWZ (description of the subject of the contract) and in Appendix No. 2 (Assertiment and price list). The contracting authority requests that the subject of the contract be carried out in accordance with the following legal acts: Act of 25 August 2006 on food and nutrition safety (consolidated text: Journal of Laws 2023, item 1448), -Regulation (EC) No 852/2004 of the European Parliament and of the Council of 29 April 2004 on the hygiene of foodstuffs, -Regulation of the Minister of Health of 17 April 2007 on the collection and storage of food samples by closed-type mass caterers (Journal of Laws No. Journal of Laws No 80 of 2007, item 545), -Regulation (EC) No 1935/2004 of the European Parliament and of the Council of 27 October 2004 on materials and articles intended to come into contact with food -Regulation (EU) 1169/2011 of the European Parliament and of the Council of 25.10.2011 on the provision of food information to consumers and met the minimum requirements set out in the CVT and the Description of the subject of the contract, including the indicated standards and recommendations. The Contractor declares that the persons indicated in § 11 sec. 5 and 6 (the person responsible for the implementation of the contract and the dietician) are employed on the basis of an employment relationship - according to the submitted offer. The Contractor declares that all persons performing the subject of the contract have a valid medical certificate for sanitary and epidemiological purposes, therefore the Contracting Authority has the right to control. The Contractor must have documents confirming the above and is obliged to provide them to the Ordering Party at any request and at any time. The subject-matter of the contract shall include: -preparation of meals, to the extent and assortment specified in the Statement, -daily delivery of meals in quantity and assortment in accordance with orders, -daily collection of post-consumer waste, in accordance with the Waste Act, -preparation of menus, -collection and storage of food samples for control purposes, -preparation of documentation of deliveries carried out, -lending, for the duration of the contract, marshmallows and platform trolleys, as in the Description of the subject of the contract, along with the provision of maintenance services. The estimated number of round-the-clock meals (breakfast + lunch + dinner) - 11 120 pcs / month.
