Poland-Siedlce: Functional support
The subject-matter of the contract is the supply of medical material for the operational unit in the Mazowieckie Provincial Hospital of St. Jana Pawła II in Siedlce Sp. z o.o. The subject-matter of the contract was divided into nine lots (packages). The detailed requirements of the subject-matter of the contract are set out in the product-price form, which is attached as Annex 2 to the ToR. In respect of lots I and IV, the Contractor shall be required to make a deposit in accordance with the product-price form within a maximum of 10 days from the date of conclusion of the contract, together with the necessary instrumentation. The deposit shall be replenished immediately, but within no more than 3 working days of receipt of the notification, in such a way that the Contracting Authority has the full range covered by the deposit. As regards package II, the Contractor will create a deposit and make the instrument available at the request of the Contracting Authority within 3 days before the planned procedure.
