1. The subject of the contract is the supply of medical equipment, specialized, disposable. The subject of the contract was divided into 82 lots. Tenders may be submitted in respect of one, several or all lots. The tender in each part should be complete and should meet the detailed requirements set out in the FTZ and its annexes. The detailed description and the manner of performance of the contract is contained in the FTZ, in particular in Chapter IV paragraph. 7 and 8, the price form constituting Annex 1a to the FTZ and the model contracts constituting Annexes 5a, 5b, 5c, 5d to the FTZ. 2. PROVISIONAL PROVISIONAL MEASURES: 1) The Ordering Party requires the submission of the following evidence together with the tender: a) catalogue cards or other equivalent means of proof in accordance with Article 106 section of the Public Procurement Act, confirming the parameters confirming the compliance of the offered equipment with the requirements of the price form - applies to parts no. 13, 22, 25, 37, 62, 68; a certificate attesting to the possession by the manufacturer of the ISO 13485 standard or an equivalent document for the medical devices offered (in the case of an equivalent standard, this information should be included in the tender) with the exception of part No 4, point. 2 and point. 3; 15; 16; 20A; 20B; 44A; 49A; a certificate attesting to the possession by the manufacturer of the ISO 14001 standard or an equivalent document for the medical devices offered (in the case of an equivalent standard, this information should be included in the tender) for parts Nos 43, 46, 70; the EU declaration of conformity for Part No 75 to confirm that the products offered are a medical device; f) The Contractor shall present the manufacturer's studies and the sources of their clinical publications on the impact of the use of a needle-free connector on the reduction of the incidence of catheter-like infection of the circulatory system in dialysis patients – for Part No 12. 2) If the Contractor fails to submit the evidence in question or the evidence in question is incomplete, in accordance with Art. 2 PZP The Employer provides for the possibility of supplementing them at the Employer's request within the time limit set by the Employer, unless, despite the submission of the measure of evidence in question, the offer is subject to rejection or there are grounds for invalidating the procedure. 3) The means of proof in question, drawn up in a foreign language, shall be submitted together with a translation into Polish. 3. The Contracting Authority provides for the use of the so-called reverse procedure referred to in Art. 1 of the PZP Act.
