Description

1. The subject of the order is the delivery of various drugs according to 56 packages to the Customer's headquarters (Kraków, Os. Golden Autumn 1), as described and required in Appendix 1 to the nin. Specifications. 2. Validity/warranty period of the subject of the contract: shall be not less than 12 months from the date of delivery and, for enteral and oral feeding products, not less than 12 months but not less than 6 months. 3. In drug and chemotherapy programs, only drugs included in the NFZ reimbursement lists are allowed (the price of the drug may not exceed the funding limit set by the NFZ). 4. Ordering party, including the form of the drug in the Specification, e.g. ‘table’ means all types of tablets, ‘capsule’ means all types of capsules, etc. In other cases, the exact form of the drug is specified, i.e. ‘intra-intestinal tablet’ means that only an intra-intestinal tablet is authorised. 5. The medicinal product which is the subject of the contract should be understood as medicinal products within the meaning of the Pharmaceutical Law of 6 September 2001 (Journal of Laws No. U. of 2024, item 686, as amended).The medicinal product offered must be authorised under the conditions set out in Article 3 or 4 para. 1 and 2 or 4a of the Pharmaceutical Law Act. 6. The Ordering Party, in the implementation of this contract in the part concerning the transport of the medicinal product to the seat of the Ordering Party, allows the participation of subcontractors with rights. 7. The other terms of the contract are set out in the draft contract, which is Annex 3 to the Specifications.
