Description

Postępowanie prowadzone jest w trybie przetargu nieograniczonego na podstawie art. 132 ustawy Pzp. Przedmiotem zamówienia są dostawy wyrobów medycznych dla Bloku Operacyjnego według poniższych części: Zadanie 1 - Zadanie 14. Szczegółowy opis przedmiotu zamówienia ww. części został wykazany i opisany w „Formularzu cenowym” (załącznik nr 2) oraz we Wzorze umowy I (załącznik nr 3) oraz Wzorze umowy II (załącznik nr 3a) stanowiących integralną część SWZ. Warunki umowne realizacji przedmiotowego zamówienia publicznego zawierają wzory umów, stanowiące integralną część SWZ. W przypadku zaistnienia podstaw do powierzenia przetwarzania danych osobowych Wykonawca zobowiązuje się do podpisania umowy powierzenia przetwarzania danych osobowych (zgodnie z załącznikiem nr 8 do SWZ). Szacunkowa wartość przedmiotu zamówienia bez prawa opcji 1 466 353,00 PLN.
