1. The subject of the contract is the supply of medical devices divided into 8 parts: Part 1 - Training defibrillator 10 sets Part 2 - Basic defibrillator 2 set. Part 3 – Clinical and transport ventilator 1 set Part 4 – neonatal resuscitation station 3 Kpl. Part 5 – Hospital ventilator with trolley 2 set. Part 6 — Simulated defibrillator 1 set Part 7 - Advanced 1 set defibrillator Part 8 - Open incubator - a stand for resuscitation of newborns 1 set. 2. ANNEXES CONCERNING THE SUBJECT MATTER OF THE CONTRACT: - Description of the subject-matter of the contract - Annex 2 to the FTZ, - Table of conformity of the offered subject-matter of the contract with the DNSH principle (Do No Significant Harm) - Annex 2a to the FTZ, - Table of assessment of warranty conditions - Annex 4 to the FTZ, - Terms of warranty, warranty and warranty service - Annex 5 to the FTZ, - Procedure for delivery and collection of the device - Annex 6 to the FTZ.
