1) Package 1:
supply of the following equipment:
(a) a two-channel magnetic field therapy unit – 1 unit,
(b) a four-channel magnetic field therapy apparatus – 1;
(C) biostimulant laser – 2 units,
(D) UV lamp – 2 units,

meeting the requirements set out in the Form for the required technical conditions set out in Annex 2.9 to the ToR, together with the installation and instructions for use, in accordance with the requirements set out in paragraph 1. 3 under the conditions set out in the Model Contract, which is attached as Annex 7 to the ToR;
10) Package 10:
supply of a set of cameras for walking analysis – 1 kpl.,
meeting the requirements set out in the Form for the required technical conditions set out in Annex 2.10 to the ToR, together with installation and instructions for use, in accordance with the requirements set out in paragraph 1. 3 under the conditions set out in the Model Contract, which is attached as Annex 7 to the ToR;
11) Package 11:
delivery of spirometer – 1 kpl.,
meeting the requirements set out in the Form for the required technical conditions set out in Annex 2.11 to the ToR, together with installation and instructions for use, in accordance with the requirements set out in paragraph 1. 3 under the conditions set out in the Model Contract, which is attached as Annex 7 to the ToR;
(12) Package 12:
supply of a wireless module to the F-Scan system – 1 kpl.,
meeting the requirements set out in the Form for the required technical conditions set out in Annex 2.12 to the ToR, together with installation and instructions for use, in accordance with the requirements set out in paragraph 1. 3 under the conditions set out in the Model Contract, which is attached as Annex 7 to the ToR.
2. A detailed description of the subject-matter of the contract for each package is set out in the Forms for the required technical conditions referred to in Chapter II of the ToR, which are Annexes 2.1 to 2.12 to the ToR respectively.
3. The Contractor will be responsible for:
(1) the supply of equipment covered by the contract to the Centre for Sports and Rehabilitation of the Warsaw Medical University at ul. Prince Trojden in Warsaw, 02-109 Warsaw, to the premises indicated by the Contracting Authority;
2) installation, understood as assembly and connection of equipment, starting it, performing calibration (if applicable) and checking the correctness of operation;
(3) provision of instructions for use;
4) the handing over of the equipment to the Contracting Authority (the User), attested by a record of receipt drawn up in accordance with the model set out in Annex 3 to the model contract.
