Description

1. Przedmiotem zamówienia jest dostawa implantów neurochirurgicznych dla potrzeb Szpitala Wojewódzkiego im. Jana Pawła II w Bełchatowie. 2. Kod i nazwa według Wspólnego Słownika Zamówień (CPV): 33184100-4 Implanty chirurgiczne 3. Opis, wielkość i zakres przedmiotu zamówienia: 1) Przedmiotem zamówienia jest dostawa implantów neurochirurgicznych dla potrzeb Szpitala Wojewódzkiego im. Jana Pawła II w Bełchatowie. 2) Zamawiający podzielił przedmiot zamówienia na 21 części (pakiety 1-21), zgodnie z opisem zamieszczonym w Załączniku nr 7 do SWZ. 3) Oferowane w postępowaniu wyroby posiadają wszystkie niezbędne certyfikaty, w tym deklaracje zgodności, są zgodne z normami obowiązującymi na terenie Unii Europejskiej, spełniają wymagania określone w ustawie z dnia 7 kwietnia 2022 r. o wyrobach medycznych oraz wymagane przepisami nadrzędnymi dopuszczenia do obrotu i używania. 4) Pozostałe wymagania dotyczące realizacji zamówienia oraz zasad współpracy między zamawiającym a wykonawcą określają projektowane postanowienia umowy w sprawie zamówienia publicznego, stanowiące Załącznik nr 3 do SWZ.
