1. The subject-matter of USK No 1, i.e. N. Barlicki w Łódź, Łódź Province, Gm. M. Łódź, is the supply of pharmaceutical products, medical devices and dressings with packaging characteristics and quantities listed in detail in Annex 1A to the specifications, which also constitutes a tender form.
2. Common Procurement Vocabulary (CPV) code:
33.60.00.00-6 – Pharmaceuticals;
33.14.00.00-3 – medical material;
33.14.11.10-4 – dressings.
3. The Procurer reserves the right to request the supply of samples for the preparations offered. The number of samples, the place and the time limit for their delivery shall be determined by a separate document.
4. The contracting authority reserves the right to restrict, including a complete suspension of the purchase of the total quantity of goods, as well as the various types and ranges thereof, during the various periods of validity of the contract and in relation to the total quantity of goods envisaged in the contract, depending on the course of treatment, changes in the profile of the services provided and changes in diagnostic and therapeutic methods. On that basis, the Contractor will have no claim other than a claim for payment for goods already delivered.
5. The payment period shall not be less than 30 days and no longer than 60 days.
6. The Contractor shall indicate a single payment deadline for all packages (tasks) submitted in the tender.
7. The subject-matter of the supply of medicinal products/medical devices/explosions will have all the authorisations and registrations required by Polish law, as well as a period of validity of not less than 12 months from the date of their delivery.
8. The contracting authority allows the number of sales packaging to be recalculated if the number of items (e.g. tablets, ampoules, kilograms) is present on the market after the Contractor has asked a question. The quantity of packages must be recalculated in accordance with the requirements of the Contracting Authority indicated in the ToR, with an accuracy of 2 decimal places (for example, if, as a result of the conversion, the number of packages will be 1.22 op., the number of 1.22 is to be valued and 1.225ops should be rounded to 1.23 op).
