Description

1. The contract was divided into the following parts (packages). The Ordering Party allows the possibility of submitting an offer for any number of packages 2. The subject-matter of the proceedings is: PACKAGE I Group life insurance service for employees and family members of employees of the Provincial Sanitary and Epidemiological Station in Rzeszów together with the District Sanitary and Epidemiological Stations of the Podkarpackie Voivodeship. PACKAGE II Group voluntary medical care service for employees and family members of employees of the Provincial Sanitary and Epidemiological Station in Rzeszów together with the District Sanitary and Epidemiological Stations of the Podkarpackie Voivodeship. The service may be provided in the form of insurance, subscription or provided by a medical entity providing medical care services. 3. The Employer does not guarantee that all employees and family members of the Employer's employees will join the group life and health insurance. During the insurance period, there may be changes in the number of insured persons. If an employee or a member of the employee's family renounces insurance, the Employer is not obliged to pay an insurance premium for them. 4.Detailed terms of the contract together with a description of the subject of the contract and the age and gender structure can be found in the annexes to the FTZ.
