Description

1. The subject of the contract is the supply of programme medicines and miscellaneous medicines for the John Paul II Provincial Hospital in Bełchatów. 2. Common Procurement Vocabulary (CPV) code and name: 33.60.00.00-6 Pharmaceutical products 3. Description, size and scope of the subject of the contract: 1) The Ordering Party divided the subject of the contract into 33 parts (Packages No. 1 - 33), as described in the Price Form according to Annex No. 2 to the SWZ. 2) The other requirements for the performance of the contract and the rules of cooperation between the contracting authority and the contractor are set out in the draft provisions of the public procurement contract, which are Annex 3 to the SPC. (3) Where the description of the subject-matter of the contract contains indications of the trade marks, patents or origin, source or specific process which characterise the products supplied by a particular economic operator, such indication shall be deemed to be accompanied by the words ‘or equivalent’. In this case, the contracting authority allows the contractor to submit a tender for an equivalent preparation, whereby such a tender consists in offering preparations identical to the subject of the contract, for which there is a common name according to the index of international names. In case of doubts of the contracting authority regarding the equivalence of the offered medicines, the contractor will be obliged to provide explanations and demonstrate their equivalence at each request of the contracting authority.
