Description

1. The subject-matter of the contract is the successive supply of disposable staples, loads for staples and sets of loads necessary for VATS and open surgery procedures at the Klara Jelska Specialist Hospital for Lung Diseases ‘Rebirth’ in Zakopane. A detailed description of the subject of the contract can be found in Annex 1 to the SWZ (price calculation of the offer). 2. Within the scope of package 1, the Contractor is obliged to provide (lend) for the duration of the contract: - the necessary instrumentation fully enabling the use of disposable tissue preparation tools referred to in items 13, 14, 15 and 16 of the table - price calculation for the offer; - reusable electrosurgical platform compatible with item 17 of the table - price calculation for the offer and instrumentation necessary to perform 12 escapees using disposable shields. Delivery of the borrower: instrumentation, electrosurgical platform - should take place within 7 days of placing the order. Receipt of the loan object will take place not earlier than one month after the date of the last delivery of sets, covers purchased on the basis of the concluded contract. 3. The object of delivery must have an entry or notification to the Register of Medical Devices, a declaration of conformity issued for them and must be marked with the CE conformity mark in accordance with the Act of 7 April 2022 on medical devices (Journal of Laws No. (Journal of Laws 2022, item 974, as amended) 4. The subject of the contract is to be delivered in the manufacturer's packaging. 5. The outer and inner packaging is to contain the name of the goods, the serial number, the date of manufacture and the expiry date. 6. The Contractor must guarantee a sufficiently long period of validity of the delivered products (minimum 1 year), counted from the date of delivery.
